Reapply [ProfileData] Implement contains in SampleProfileNameTable (NFC) (#213408)
This patch implements contains(StringRef) and contains(uint64_t) in
SampleProfileNameTable and SampleProfileReader to serve symbol
membership queries directly from the reader -- "is this symbol in the
name table?".
Without this patch, users of the sample profile reader, namely
SampleProfileLoader::doInitialization and SampleProfileNameSet, each
construct their own StringSet<> containing all name table entries.
That is, we end up with two instances of StringSet<> with identical
contents. Since these instances hold their own copies of symbol
strings on the heap, both the constructor and destructor take up a
large portion of compilation time.
This patch teaches SampleProfileReader::contains to directly serve
symbol membership queries.

mincore(2): Fix several bugs

This change should fix a crash in Mesa where it calls mincore() to
determine whether a protocol field is a valid pointer in order to
keep compatible with an old protocol version.
* Fix an off-by-one error in filling the skipped entries. lastvecindex
refers to the last valid byte, so the new bytes should come after it.

mincore(2): Add 'vm.mincore_mapped' sysctl to control reporting
Add the 'vm.mincore_mapped' (default ON!) to control whether to report
only the mapping status (if set to a non-zero value) or the physical
residency (if set to a zero value; the old behavior).
The idea of implementing this and enabling it by default is to help
mitigate page cache attacks [1].

nfs_commonkrpc.c: Handle NFSERR_DELAY for Sequence correctly
Unlike RFC5661 (the original NFSv4.1 RFC), RFC8881 specifies
that a NFS4ERR_DELAY reply to the SEQUENCE operation requires
a reply using the same slot/sequence#.
This patch fixes handling of this case, so it conforms to
RFC8881.
